Journey through Culture Change
General Course Overview
  1. This course has been developed as an 24 CE hour course to be taught on separate occasions.  Taken in its entirety, this course contains 7 Ethics hours for Texas LNFA's.  The topics are:
    1. Taking Institutional Care to Individualized Care
    2. Pioneering Culture Change
    3. Regulatory & Operations Issues
  2. This course may be taken several different ways depending on the number of hours needed for license renewal.  The fee is $498 for all 24 hours.  For any combination less than 24 hours, the fee is as follows:
    1. 16 hours - $340 
    2. 8 hours - $175
    3. 4 hours - $129 
  3. This course will be taught by Video Conference over the Internet. You will need a computer with a high-speed Internet connection, a sound card and speakers.  You will be able to interact by using an Instant Messaging feature within the broadcast.  A telephone conference may be used.

Taking Institutional Care to Individualized Care

This 8 hour program is the foundation ~ the “WHY” of Culture Change. CMS conducted a 254 Nursing Home Research Study that clearly illustrates the value of Individualized Care. CMS is clearly supporting Culture Change. This exciting video series captures the Regulatory Side of Culture Change. You will get caught up on the concepts of Culture Change and be able to position your facility to succeed in your Culture Change Journey. Use this course to build the foundation for implementing your Culture Change journey.
Integrating Individualized Care | 4 CE hrs
This video series provides a historical perspective on the OBRA 87 mandate that each nursing facility ‘provide care and services to attain or maintain the highest practicable physical, mental and psychosocial well being of each resident’, and demonstrates how nursing homes achieve better clinical outcomes when care delivery systems are individualized.
Implementing Individualized Care | 4 CE hrs
This video series focuses on the ‘How’ of Culture Change. We will look specifically at leadership and workplace practices to identify essential practices of exemplary leaders and key practices that contribute to low turnover: the importance of individualized care to good clinical outcomes, focus on concrete leadership and workplace practices that allow facilities to provide individualized care, stabilizing staffing that builds relationships leading to consistency in care and the result of a positive impact on staff, residents and families.
Pioneering Culture Change

This 8 hour program is the walls of your house ~ the “WHAT” of Culture Change. This material was developed by the Galichia Center on Aging at Kansas State University in Manhattan Kansas.  If you are actively pursuing, or just thinking about pursuing a Resident-Centered Care Facility, then this seminar will take you through the necessary concepts for implantation in your facility. These concepts include: (1) Intro to Culture Change, (2) Measuring Change, (3) Returning Control to Residents, (4) Family and Community, (5) Strengthening Staff, (6) Creating Home, (7) End of Life Care, (8) Activities and, (9) Dining.  Use this course to build the walls of your Culture Change journey.
Components, Part I | 4 CE hrs including 1 Ethics hr
For Culture Change to advance it will need support from boards and owners. This support is more likely to occur if Culture Change factors can be established as having provided a cost savings as well as improvements in the quality of life. Topics include the (1) definition of Culture Change, (2) how to measure Culture Change and explore the areas of Culture Change such as (3) returning control to residents, (4) importance of family and community in long term care and (5) how to strengthen your staff.
Components, Part II | 4 CE hrs including 1 Ethics hr
Continue to explore the areas of Culture Change such as (6) creating a home-like atmosphere for residents, (7) the issues of caring for residents at the end of their lives, (8) having meaningful activities and (9) how to change the dining experience into a ‘sit down meal’.
Regulatory & Operations Issues

This 8 hour program is the roof on your house ~ the “HOW” of Culture Change. Learn how the future survey process will be affected by Culture Change concepts, understand what you are now doing that is Culture Change, and finally develop a map for your journey. This will IMPROVE your Staff Engagement, Motivation, Trust, Ownership, and Profits by REDUCING your Stress, Conflict, Absenteeism, Staff Turnover, and Disputes.  Use this course to put the roof on your Culture Change journey.
F-Tags Crosswalk to Culture Change | 4 Ethics Hr
Are you ready for the surveyors? In this session you will learn how the surveyors will be looking at your facility for Culture Change. We will examine over 150 F-Tags from the Federal regulations, guidance to surveyors and survey protocols. You will be able to use this information to provide consistent quality care for residents and be able to implement Culture Change programs.
Measuring Your Journey | 4 CE hrs including 1 Ethics Hr
Where are you now? Where do you want to be in 3 months, 6 months, 1 year? In this session we will use two surveys, Culture Change Indicators and Artifacts of Culture Change, to discover where you are today in your journey, where you want to go and how you can sequence and implement your journey in a cost effective manner. These two tools will assist you in measuring your progress during your Culture Change journey.
