Our Policies Reschedule Policy
CHA Seminars recognizes that from time to time there are unforeseen circumstances that will prevent you from attending a seminar for which you have enrolled and paid the pre-registration fees to attend. Please notify CHA Seminars as soon as possible that you will not be able to attend a seminar for which you have registered.
It is CHA Seminars' policy to issue credit in lieu of a refund. We will automatically issue a credit for any unattended seminar - hour for hour. This credit is valid for 24 months from the date of the missed seminar. CHA Seminars will honor all requests to reschedule a seminar. The Reshedule fee is $25. Those individuals requiring a refund will be issued a check less a 50% Administrative fee. Types of Payments

Please note that our Registration Forms are in PDF format. If you are not on a high speed internet connection, it may take longer for our form to load. Please be patient. If you do encounter problems, please call 512.336.1005 and we will take your registration over the phone.
CHA Seminars uses FAX registration as a method to ensure protection of your sensative data. Currently our website is not a secured website that would allow for the transmission of sensative data. Early Bird / Pre-Registration Fees
Please help us help you keep your continuing education costs down by utilizing our Early Bird / Pre-Registration process.
It's Easy ~ Just call in or fax your Registration Form to us by 5:00pm CST two (2) days prior to the date of the seminar you wish to attend. You will receive an email confirmation of your Registration within one (1) business day. Walk-In / Late Registration
CHA Seminars accepts registrations for seminars right up to the start time of each seminar.
A Late Fee of $15 per 4-hour seminar will be added when registering at the seminar. Certificate of Completion
We issue one Certificate of Completion for all courses completed during a license renewal period. Your Certificate of Completion will be emailed to you within one week of the completion of each seminar. Your certificate will contain the following information as needed on the DADS Continuing Education Record for license renewal:
Replacement Certificates
Requests for replacement certificates or verification of attendance must be made in writing. Please complete the Request for Replacement Certificate and fax to 512.336.1008. Requests will be processed within 14 days. There is a $25 fee for each replacement certificate or to verify attendance at a seminar. There is an additional fee of $25 for RUSH processing.
Requests to fax certificates to the Board are considered a 'replacement certificate' and fees apply accordingly. Payment Statement Policy
This certificate is issued only upon (1) the completion of the requirements governing the license of the person whose name is listed on the certificate and (2) payment of funds as agreed on the Registration Form. If for any reason transactions are disputed and funds are returned to the holder of the accounts, CHA Seminars will revoke said certificate for nonpayment of funds and notification will be made to the Texas State Board of Licensure for Nursing Facility Administrators, and/or any other Licensing Agency required to monitor Continuing Education, that the certificate for the seminars in question were obtained under fraudulent means and the certificate has been revoked. |
